Napa County Public Health Division was charged with developing a complex planning process to identify health needs and develop a road map for change. Working closely with then Public Health Officer, Karen Smith, and other staff, Kym Dorman (while at Harder+Company) led the countywide Mobilizing for Action through Planning and Partnerships (MAPP) process for a consortium of public, private, and nonprofit organizations in Napa County over a two-year period. The process consisted of three components: developing a comprehensive Community Health Assessment (CHA) that included collecting, analyzing, and interpreting community data to identify community health issues, strengths, and needs; developing a Community Health Improvement Plan (CHIP) to build on strengths while addressing the health issues and needs identified in the CHA; and finally, developing the Community Health Action Plan (CHAP) to identify strategies for each collaborative partner to carry out the work along with metrics to track progress. Kym planned and facilitated numerous community meetings with approximately 100 community members and stakeholders every quarter for two years. The resulting products are helping to align community efforts and position the Napa County Public Health Division for accreditation by the national Public Health Accreditation Board (PHAB).

  • Oversaw extensive qualitative and quantitative data collection process, including collecting 2,383 surveys in English and Spanish, reviewing secondary data, and conducting 28 workshops with 300 people.
     

  • Developed cross-sector buy-in for the community health assessment (CHA), community health improvement plan (CHIP) and more specific community health action plan (CHAP) with specific strategies and metrics for each participating organization.
